2021-2022 Financial Aid Forms


Our office requires students to complete and submit their financial aid forms online. This prevents you from having to complete a paper form and bring it in to the office. When you sign in to your MySCF account online and click on the appropriate form, you will be taken directly to that form and be able to complete and submit it immediately. Our office will receive the document and have it processed within 7 – 10 business days. This system makes the process of submitting financial aid documents much easier and more efficient. If you experience any difficulties, please feel free to contact our office. 

You are able to view your financial aid requirements by completing the following steps:

  1. Log into MySCF
  2. Click on the Student Finances tab
  3. Check the FInancial Aid Requirements channel to see if any documents are required.

Required Verification Forms

**Please do not submit documents unless requested by the Financial Aid Office**

The following must be completed in-person at the Financial Aid Office:

  1. High School Completion/Statement of Educational Purpose (Independent)
  2. HighSchoolCompletion/StatementofEducationalPurpose(Dependent)
  3. High School Completion/Verification of Household/Statement of Education Purpose (Independent)
  4. High School Completion/Verification of Household/Statement of Education Purpose (Dependent)

Please be sure to have a government issued ID with you, when completing this form in the Financial Aid Office. Examples: State Driver's License, State ID Card, US Passport, Permanent Resident Card.

    • DOCUMENTATION MUST BE UPLOADED. Incomplete appeals will be automatically denied.
