Tutorial Assistance Forms

Students eligible for benefits under Chapters 30, 32, 33, 1606, and 35 may be eligible for tutorial assistance from the VA. To receive the benefit, a student must be enrolled on at least a half-time basis (6 semester hours) or greater and must show deficiency in a course or courses required for their degree program.  The VA pays a maximum monthly payment of $100. The maximum total benefit is $1,200. To apply, students must complete VA form 22-1990t, Application and Enrollment Certification for Individualized Tutorial Assistance, and State College of Florida Tutorial Assistance Form.

Authorization of Tutorial Assistance form.Any Veteran/Dependent who wishes to gain the use of a tutor must complete the forms listed above.  They may be obtained from Katherine Snyder in the Financial Aid office on the Venice campus, Student Services Bldg 100, Room 160; or Cecile Lansiquot or a Work Study in the Financial Aid office on the Bradenton campus, Korcheck Bldg 1, Room 137.
