2013 Health Insurance Options
Employees may select from four types of health plans administered by Florida Blue (formerly known as Blue Cross Blue Shield of Florida)..
Employee coverage
- Minimal or no cost to employee, depending on the plan selected.
Dependent/Partner coverage
- Premium paid by employee
- Cost based on the plan you select and who you cover

Plan 1 - PPO Economy Plan (3566) - BlueOptions - This is a preferred patient organization (PPO) plan that allows you to select health care providers from within or outside of a provider network. You will receive the greatest benefits when you obtain services from providers within NetworkBlue. This plan is offered with no cost to the employee, has a higher deductible, and has lower premiums for dependents.
Plan 2 - HMO Plan (10) - BlueCare - This is a managed care plan (HMO) through Health Options, Inc. You must select a primary care physician at the time of enrollment. All medical care must be coordinated through your primary care physician.
Plan 3 - PPO Plan (3562) - BlueOptions - This is a preferred patient organization (PPO) plan that allows you to select health care providers from within or outside of a provider network. You will receive the greatest benefits when you obtain services from providers within NetworkBlue.
DV - Dental / Vision - This is a supplemental plan for employees who already have adequate health insurance coverage. There is no option for dependent coverage. The plan includes the high dental plan as well as a vision discount only.
BlueMedicare Group PPO Coverage - This is a Medicare Advantage plan which coordinates with your Medicare insurance. You must have both Medicare A and Medicare B coverages to participate. This plan is available to Medicare-eligible retirees only. Please contact Human Resources for an application. 941-752-5393
